Job SummaryWe are seeking an experienced Advanced Practice Provider (APP) to join our team of compassionate healthcare professionals. As a key member of our geriatric care team, you will provide primary, urgent, and acute medical care to seniors in their homes, working collaboratively with our physicians, social workers, and care team coordinators to deliver concierge-level care.
Responsibilities- Provide primary care to patients in their homes, including routine check-ups, urgent care, and acute medical interventions.
- Collaborate with our interdisciplinary team to develop and implement comprehensive care plans that address the unique needs of each patient.
- Conduct initial patient screenings, physical examinations, and create appropriate assessments and plans.
- Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options.
- Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents.
- Update patient records and ensure accuracy at each patient appointment.
- Deliver quality care while maintaining all company metrics pertaining to documentation, coding, HEDIS measures, patient engagement, and medical expense management.
- Serve as a resource for patients in delivering ongoing care information, counseling, and guidance for critical conditions.
- Refer to internal and external resources as needed, such as WellBe behavioral health protocols.
- Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings.
- Maintain required licensure and certification.
- Deliver palliative care and end-of-life care to patients and families as needed.
- Master's degree in Nursing.
- Valid nurse practitioner license in the state of operations and current board certification in your field.
- 5+ years of progressive care experience.
- BLS and ACLS certification, and an NPI# as an NP.
- Valid driver's license, car insurance, access to a car, and willingness to drive to patient homes/patient locations.

This is a full-time, permanent position, with a flexible schedule and on-call rotation. Travel requirements may be up to 100% locally. Ability to lift up to 20lbs, with occasional lifting of up to 50lbs, and ability to stand for extended periods. Fine motor skills/visual acuity required.
