Job Summary

We're seeking a skilled Advanced Practice Provider to join our team of compassionate caregivers. As a key member of our interdisciplinary team, you'll work closely with physicians, social workers, and other healthcare professionals to deliver comprehensive care to our patients.

Responsibilities
  • Provide primary care to patients in their homes, including routine check-ups, urgent care, and chronic disease management.
  • Collaborate with our team to develop and implement individualized care plans that address the unique needs of each patient.
  • Conduct thorough assessments, create accurate diagnoses, and develop effective treatment plans.
  • Prescribe and manage medications, as well as order and interpret diagnostic tests.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care coordination.
  • Participate in regular team meetings to discuss patient progress, share best practices, and address any challenges.
Requirements
  • Master's degree in Nursing and current board certification as a Nurse Practitioner.
  • 5+ years of progressive care experience, with a focus on geriatrics, internal medicine, and palliative care.
  • Valid driver's license, car insurance, and willingness to drive to patient homes.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
