Job Summary

We are seeking a skilled RN Case Manager to join our team at St. Peter's Hospital. As a key member of our healthcare team, you will play a vital role in managing the progression of patients' care and ensuring optimal communication and patient flow across the continuum.

Responsibilities
  • Collaborate with the healthcare team to develop and implement individualized care plans that address the unique needs of each patient.
  • Engage patients and their families in the care planning process, ensuring that their voices are heard and their concerns are addressed.
  • Utilize evidence-based practice guidelines to inform care decisions and ensure that patients receive the highest quality care.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless transitions and optimal patient outcomes.
  • Provide education and support to patients and families to promote self-management and independence.
  • Participate in quality improvement initiatives to enhance the delivery of care and improve patient outcomes.
Requirements
  • Current and valid NYS RN license.
  • Bachelor of Science in nursing or willingness to pursue a defined plan for matriculation in a program demonstrated by ongoing coursework each semester.
  • 2 years of experience in an acute setting.
Preferred Qualifications
  • 1+ year of experience in case management or utilization management.
  • Case Management certification (CCM) or Certified Professional in Healthcare Management (CPHM).
  • Discharge planning and community resource knowledge.
  • Maintain the highest level of customer service while completing day-to-day duties and responsibilities.
  • Critical thinking skills to problem-solve.
  • Excellent time management and organizational skills.
  • High level of efficiency to communicate in a fast-paced work environment across people and teams of different backgrounds and demographics.
  • Engage within a team setting to help promote a strong culture.
  • Passion to learn, grow, and develop in the nursing field.
