Healthcare Case Manager RN

Transforming Patient Care with Compassion and Excellence
CVS Health is seeking a dedicated and compassionate RN Case Manager to join our team. As a key member of our Dual Eligible Special Needs Plan, you will play a vital role in elevating patient care to new heights.
Key Responsibilities:
- Collaborate with members, providers, and community organizations to address complex health and social challenges.
- Develop proactive plans of care to enhance short and long-term outcomes.
- Conduct assessments, using clinical tools and information/data review, to evaluate member needs and benefits.
- Apply clinical judgment to reduce risk factors and barriers, and address complex health and social indicators.
- Collaborate with supervisors and stakeholders to overcome barriers and achieve goals.
Requirements:
- Minimum 2+ years of Case Management, Discharge Planning, and/or home health care coordination experience.
- Bilingual desired (English/Spanish or English/Russian).
- Effective computer skills, including navigating multiple systems and keyboarding.
- Demonstrates proficiency with standard corporate software applications.
