Job Description

Consejo Counseling and Referral Service, a non-profit organization, is seeking a full-time System Navigation Case Manager to join our team.

Key Responsibilities:

  • Work with clients of diverse age ranges and backgrounds, addressing persistent mental health concerns and complex needs.
  • Provide responsive and comprehensive services to improve clients' stability, wellness, and recovery.
  • Assist in making referrals, coordinating care, and working with multiple systems, including jails, hospitals, vocational and housing programs, schools, legal systems, and medical providers.
  • Offer system navigation services to 15-24 year old young adults harmed by the criminal justice system living in Seattle/King County.
  • Provide services at home, school, community, detention centers, and other facilities where client services are needed.

Essential Duties and Responsibilities:

  • Work in partnership with clients and other providers to provide linguistically and culturally sensitive services.
  • Manage a caseload of 35 active clients at all times.
  • Provide direct services daily, including case management, individual services, group services, phone services, care coordination, etc.
  • Document all services properly at Consejo records.
  • Conduct outreach and engagement efforts.
  • Develop marketing materials to reach the target population.
  • Provide advocacy, outreach, information, and referral services to low-income clients in need of case management services.
  • Serve clients with criminal justice involvement, gang involvement, etc.
  • Ensure clients in need of SUD and MH services are connected to Consejo treatment services.
  • Coordinate services with other internal departments and service providers to ensure optimal client care.
  • Provide youth and their families with communication skills, tools, and other resources to support recovery.
  • Provide early intervention and education, assistance to overcome barriers in transitioning out, mentoring opportunities, coordination guidance, re-entry plan development, and youth transitioning out of detention centers.
  • Assess crises and provide crisis intervention when necessary.
  • Establish liaisons to access community services, including medical care, housing, food, vocational programs, and GED resources.
  • Provide education, outreach, and information in the community as needed, including services in juvenile detention facilities.
  • Promote healthy living techniques for families to model/share within their communities.
  • Serve as a liaison between the client and multiple systems utilized by the client.
  • Advocate on behalf of the client to ensure they remain in school, home, and in the community when appropriate.
  • Work in partnership with law enforcement, probation department, department of corrections, and judicial system to reduce the risk of re-offense or continued criminal justice involvement.
  • Create an individualized plan for each client to monitor their progress and service success.
  • Use all available tools to keep clients engaged in services.
  • Reduce clients' law enforcement involvement, truancy, school suspension, juvenile detention/jail, psychiatric hospitalization, out-of-home placement, and homelessness.
  • Complete all required documentation to ensure 100% file compliance.
  • Attend mandatory clinical in-services/trainings, supervision, and regularly scheduled department/agency meetings.
  • Fulfill other duties as assigned to meet agency operations' needs.

Recovery Best Practices:

  • Understand and apply the ten core principles from the national consensus statement on recovery (hope, self-direction, individualized and person-centered, empowerment, holistic, non-linear, strengths-based, peer support, respect, and responsibility).
  • Conduct strengths-based interviews and utilize strengths in implementing the care plan.
  • Provide outpatient evaluations, assessments, care plans, and progress notes that are individualized and recovery-oriented.
  • Assist clients to assume or resume employment, education, or normalized housing, and to thrive in these roles.
  • Locate community resources outside of the mental health system and inspire consumers to use them.
  • Provide culturally competent recovery practices for Latino clients.
  • Implement a Wellness Recovery Action Plan.
  • Use motivational interviewing skills to enhance recovery potential.
  • Help people who have experienced trauma to recover.
  • Provide family psycho-education and engagement.

Qualifications:

  • Bachelor's degree with major specialization in criminal justice, social, behavioral, or human services-related fields or High school diploma or GED with related volunteer experience comparable to two years full-time work in a social, behavioral, or human services or related fields.
  • Valid WA state driver's license at the time of employment, able to safely operate a motor vehicle, and have reliable transportation.
  • Proficient in speaking, reading, and writing Spanish and English.
  • Eligible to work in the United States of America and provide documents that support eligibility.
  • Sign an oath of confidentiality covering all patient-related information and serve as the designated agency privacy officer.
  • Pass a Washington State criminal background check and pass two federal fraud and abuse exclusion checks.
  • Obtain an agency affiliation counselor certification.

Work Schedule:

This full-time in-person position works a typical 8 hours a day between 8 AM - 5 PM, Monday to Friday.

Salary Range:

$45,000 - $55,000 a year depending on experience, qualifications, and credentials.
