Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at the YMCA of Greater Seattle. As a Case Manager, you will provide critical support to clients enrolled in our Permanency Programs, including traditional/long-term foster care, extended foster care, therapeutic foster care, intensive in-home stabilization services, and outpatient mental health.

Key Responsibilities
  • Provide case management and care coordination services to clients, ensuring they receive all required screenings, assessments, services, interventions, and opportunities to meet their physical, psychological, social, emotional, relational, developmental, educational, independent-living, housing, and permanency needs.
  • Maintain a caseload size in accordance with WAC and Contractual requirements and guidelines.
  • Ensure clients' transitions are well coordinated and facilitated, including placements and discharges, and complete all required intake, service planning, and discharge/exit documentation in a timely and effective manner.
  • Provide leadership and guidance in the placement referral process and ensure smooth case transfers.
  • Ensure clients' parent/child/sibling visitation needs and requirements are met, and natural supports and life-long connections are developed and stabilized.
Requirements
  • Bachelor's degree in psychology, social work, or a related social services field or equivalent experience, with a Master's degree preferred.
  • Experience working with children and/or teens in high-stress or crisis situations.
  • Ability to teach effective independence skills for a variety of learning styles and plan supportive activities.
  • Strong youth engagement and relationship-building skills.
  • Strong verbal and written communication skills.
  • Ability to transport self and client(s) in a safe and effective manner, with proof of adequate vehicle insurance coverage required.
  • Current state-approved First Aid and CPR certification and approved HIV blood-borne pathogens training.
