Overview:
As a Care Coordinator, you will manage a designated caseload, ensuring timely assessment, planning, implementation, and evaluation of discharge plans and care transitions throughout the continuum of care.

Key Responsibilities:
- Oversee daily patient assessments and monitor census to ensure all patients receive appropriate evaluations.
- Facilitate care coordination, discharge planning, consultations, advocacy, and education.
- Maintain effective communication with healthcare team members regarding assessment findings and discharge planning needs.
- Stay informed about benefits, reimbursement guidelines, and applicable regulations, providing education to patients and families as necessary.
- Collaborate with Utilization Management partners to enhance negotiations and prevent denials with payers.
- Document assessment findings and discharge arrangements in a timely manner, adhering to departmental guidelines.

Qualifications:
- Education: BSN or MSW required.
- Experience: Minimum of 3 years in a relevant field.
- Certification: Case Management Certification (ACM, CCM, or ANCC) required within 3 years of hire. Current or compact RN licensure in the state of NC is necessary if holding a BSN.

Skills and Abilities:
- Ability to work independently and manage multiple tasks effectively.
- Strong problem-solving skills for complex issues.
- Excellent written and verbal communication skills.
- Basic computer proficiency.

Quality of Life:
