Job Description

We are seeking a dedicated Registered Nurse to join our team as an Elderplace RN Specialist. In this role, you will work closely with our interdisciplinary team to provide care management, care coordination, and skilled care services for at-risk frail elder patients.

About the Role
  • You will be responsible for planning, overseeing, and monitoring all nursing and unlicensed caregiver services to each patient on your panel within the context of an IDT.
  • You will travel between multiple locations to provide care and support to our patients.
Requirements
  • Oregon Registered Nurse License
  • National Provider BLS - American Heart Association
  • Relevant certifications applicable to the position, as specified by the Nurse Manager
