Nursing Case Manager Position with Competitive Compensation

About the Role
This position involves providing care management and coordination services for at-risk patients, addressing complex medical and psychosocial issues. You will be a core member of our patient-centered medical home (PCMH) teams led by physicians/providers, leveraging population-based, chronic care management approaches and applying clinical protocols to ensure optimal outcomes.
**Responsibilities:**
- Deliver high-quality care management and coordination services to at-risk patients
- Collaborate with multidisciplinary teams to develop and implement effective care plans
- Utilize evidence-based clinical protocols to address complex medical and psychosocial issues
- Maintain accurate and up-to-date records of patient information and progress
- Participate in regular team meetings and educational sessions to enhance knowledge and skills
Requirements and Qualifications
To succeed in this role, you should possess a strong foundation in nursing, including:
- Graduation from an accredited school of nursing
- Oregon Registered Nurse License upon hire
- 3 years of clinical experience as an RN
- Preferred qualifications include:
- Bachelor's Degree in Nursing
- 2 years of Home health, skilled nursing facility, and/or community health nursing experience
- 2 years of Managed care experience and/or utilization management experience
- 2 years of Care/Case Management experience
Compensation and Benefits
We offer a competitive salary range of $45.22 - $71.58 per hour, which may be supplemented by shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
