Job Summary

Inpatient Care Managers at Kaiser Permanente are Registered Nurses who ensure patients receive the right care at the right time. They provide quality, cost-effective clinical coordination and care management in acute care and emergency settings. Our Care Managers assess patient needs, develop and implement plans of care, and coordinate internal and community resources to support optimal healthcare utilization.

Key Responsibilities
  • Coordinate post-discharge patient care needs to ensure timely and effective discharge from the hospital setting.
  • Conduct thorough, timely, and age-appropriate patient assessments to identify care needs and health plan benefits.
  • Communicate with patients, families, and healthcare teams to develop mutually agreeable plans of care that optimize resource utilization.
  • Facilitate resolution of issues that present barriers to safe transfers through patient/team care conferences.
  • Ensure systematic and ongoing contact with interdisciplinary staff and continuing care services to ensure safe patient transitions.
  • In collaboration with the healthcare team, ensure regulatory and compliance standards are met.
Requirements
  • Minimum of two years combined RN experience in Med/Surg, ICU, Emergency Department, Home Health, Skilled Nursing Facilities, Hospice, Long Term Acute Care, or Inpatient Rehab.
  • Successful completion of an RN program by date of hire.
  • Registered Nurse License (Washington) within 6 months of hire or Compact License: Registered Nurse within 6 months of hire.
  • Basic Life Support within 3 months of hire.
Preferred Qualifications
  • Knowledge of appropriate utilization of acute hospital and Kaiser Permanente internal resources.
  • Knowledge of Medicare and Medicaid regulations related to eligibility requirements.
  • Demonstrated clinical judgment and customer-focused service skills.
