Job Title: Clinic Registered Nurse

We are seeking a highly skilled and compassionate Clinic Registered Nurse to join our team at Neighborhood Health Center. As a Clinic Registered Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The Clinic Registered Nurse will provide comprehensive nursing care to patients in a Primary Care Clinic setting. This role requires the ability to apply the nursing process (assessment, diagnosis, planning, implementation, and evaluation) to all care provided. The successful candidate will work collaboratively with the healthcare team to ensure seamless patient care and will be responsible for performing comprehensive health assessments, triaging minor to emergent situations, and responding to medical emergencies.

Key Responsibilities:
  • Perform comprehensive health assessments of patients, identifying actual and potential health needs based on medical, mental/emotional, financial, and social factors.
  • Triage minor to emergent situations and respond to medical emergencies.
  • Administer medications and immunizations as ordered by an authorizing NHC provider or via NHC approved standing order.
  • Assist patients to maintain maximum quality of life by coordinating services of community agencies and encouraging preventive practices.
  • Perform RN-led patient visits for health maintenance, patient education, and treatment of acute issues based on protocols and provider orders.
  • Consult and coordinate with health center team members to develop and implement comprehensive patient care plans based on assessment.
  • Assist patients to establish short and long-term goals to obtain optimum level of health through advocacy, counseling, teaching, problem-solving, and selection and utilization of resources.
  • Instruct individuals, families, and other groups on topics such as health education, disease prevention, and develops health improvement programs.
  • Monitor, plan, implement, and evaluate case coordination and management for patients with chronic illnesses.
  • Participate in team huddles to review the schedule at the start of the workday.
  • Participate in local vaccine management.
  • Accurately, concisely, and succinctly document in the medical record all patient contact.
  • Manage an electronic in-basket of incoming and outgoing patient calls, MyChart messages, refill requests, and other messages that directly impact patient care plans.
  • Implement required nursing procedures consistent with NHC procedures/protocol.
  • Participate in process and quality improvement workflows.
  • Remain calm and effective in high-pressure and emergency situations.
  • Assist in new employee training, as needed.
  • Maintain punctual and predictable attendance.
Requirements:
  • Associate Degree in Nursing with 2 years of nursing experience. A Bachelor's Degree of Science in Nursing is preferred.
  • Completion from an accredited school of nursing.
  • 1-3 years related experience and/or training preferred in the FQHC or primary care field.
  • Bilingual proficiency in English and Spanish preferred.
License and/or Certification Requirements:
  • Current, unrestricted Oregon RN license.
  • Active CPR certification required.
  • National Provider Identifier (NPI) required.
Knowledge, Skills, Abilities & Behaviors:
  • Ability to work independently to the full ability of RN licensure.
  • Knowledge of the principles, practices, and procedures of community health nursing, managed care, and consultation, including socioeconomic and cultural factors, methods, and techniques of crisis intervention.
  • Ability to communicate professionally and effectively both orally and in writing with a service-oriented approach, sometimes in stressful circumstances, with patients, coworkers, and the public.
  • Must be able to display sensitivity to the population and patients served.
  • Ability to use good judgment in difficult situations.
  • Ability to proficiently use EPIC Electronic Health Record.
  • Ability to comply with all HIPAA, OSHA, and Neighborhood Health Center policies and procedures.
  • Ability to learn and adapt to changing information and/or policies and procedures.
  • Ability to enter in data accurately with attention to detail.
  • Knowledge of customer service principles and practices.
  • Ability to work in a fast-paced environment.
  • Must be able to demonstrate a record of reliable attendance and punctuality.
Working Conditions:
  • The noise level in the work environment is usually moderate to loud based on patient volume.
  • As a result of high patient volume, this position experiences stressors that come along with a fast-paced environment.
  • Must be able to travel during the workday and/or cover at other locations as needed.
  • While performing the duties of this job, the employee is occasionally exposed to toxic chemicals or blood-borne pathogens and regularly handling contaminated sharp instruments.
