Physician - Primary Care Specialist

CrescentCare is seeking a skilled Physician to join our team as a Primary Care Specialist. As a key member of our healthcare team, you will provide high-quality patient care, work collaboratively with other healthcare professionals, and contribute to the development of our healthcare services.
Responsibilities- Provide comprehensive primary care services to patients, including evaluation, diagnosis, and treatment of various medical conditions.
- Work collaboratively with other healthcare professionals, including nurses, therapists, and specialists, to ensure comprehensive patient care.
- Develop and implement individualized treatment plans for patients, taking into account their unique needs and circumstances.
- Stay up-to-date with the latest medical research, guidelines, and best practices in primary care, and apply this knowledge to improve patient outcomes.
- Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination and patient satisfaction.
- Participate in quality improvement initiatives and contribute to the development of our healthcare services.
- Successful completion of an ACGME or AOA-accredited residency in internal medicine, pediatrics, family medicine, or other specialty service provided by CrescentCare under its scope of services as a FQHC and defined on the HRSA Form 5A.
- Admitting privileges with a hospital directly or a collaborative agreement with a physician who has admitting privileges.
- Maintenance of BLS certification.
- Maintenance of Board Certification in their area of practice (IM, IM/Peds, or Family Physician).
- Current Louisiana Medical Licenses without restrictions.
