Position Summary for Registered Nurse Case Manager (RNCM):
We have openings for Registered Nurse Case Managers in various locations. Your responsibilities will include:
- Planning, organizing, and directing home health care services, including initial evaluations, pain management, and care plan development.
- Ensuring a high-quality patient experience and positive health outcomes through comprehensive and personalized care plans, with proactive adjustments based on patient needs.
- Preparing complete and timely clinical documentation, coordinating care with an interdisciplinary team, and collaborating with primary care physicians to maintain continuity of care.
- Providing education and coaching to patients and their caregivers, empowering them to manage their conditions effectively and improve their quality of life.
Minimum Qualifications for Registered Nurse Case Manager (RNCM):
Candidates must possess:
- A current California Registered Nurse (RN) license
- A valid CPR certification for Healthcare Providers
- A minimum of one year of experience as a Registered Nurse (RN) in Home Healthcare is preferred
Additionally, candidates must be licensed drivers with a reliable, insured vehicle that meets state and organizational requirements. Strong observational, problem-solving, and communication skills are essential, along with the ability to work independently with minimal supervision.
