Position: Assistant Director of Nursing

Role Summary:
The Assistant Director of Nursing is responsible for managing and directing all facets of patient care for home care clients on a daily basis. This role ensures the provision of high-quality patient care through professional assessment, intervention, evaluation, and supervision of the entire patient care team, in collaboration with the Director of Nursing.
Reporting To: Director of Nursing, Compliance Officer
Qualifications Required:
- Minimum of 3 years of nursing experience, preferably in hospice care.
- Previous experience in a management role.
- Possession of a current and unrestricted nursing license; LPN or RN (RN preferred).
- Ability to work effectively within an interdisciplinary team as well as independently.
- Demonstrated capability to assist and respond to the needs of terminally ill patients.
- Strong communication skills, both written and verbal.
- Flexibility to work varied schedules.
Key Responsibilities:
- Ensure that all clinical services provided align with hospice care standards and comply with established policies and procedures.
- Coordinate and supervise the overall delivery of home care services for hospice patients and their families.
- Maintain clinical records in accordance with state, federal, and agency regulations.
- Assist in the admission process and serve as the primary clinical contact for the Clinical Administrative Assistant.
- Provide clinical expertise and support to team members as needed.
- Facilitate documentation for patient certification, re-certification, and discharge processes within the EMR system.
- Conduct routine visits to support ongoing patient care needs, including admissions and re-visits.
- Perform information visits and gather patient data for evaluating new admissions, ensuring all consents and legal documents are completed appropriately.
- Assist in administrative tasks related to the on-call nursing schedule.
- Provide written evaluations of staff performance as necessary.
- Monitor and adjust caseloads for clinical staff and oversee weekly visit schedules in collaboration with the Director of Nursing.
- Facilitate healthcare and national recognition initiatives for patients as directed by the Director of Nursing.
- Support volunteer activities as required.
- Manage all DME-related activities and patient coordination.
- Update and maintain physician and facility profiles within the EMR database.
- Assist in the orientation and training of AIDEs and LPNs, and support RN onboarding in conjunction with the Director of Nursing.
- Coordinate support from each IDG member as appropriate.
- Maintain compliance with IDG documentation requirements.
- Ensure effective communication among staff, patients, and families.
- Uphold standards of ethical business and clinical practice.
- Track and document incidents for review by the Director of Nursing.
- Ensure compliance with federal and state regulatory agencies and other certifying bodies.
- Address grievances and complaints from patients or families to the appropriate personnel.
- Report any allegations of patient abuse or misappropriation of property.
- Respond appropriately to emergency situations and ensure patient safety.
- Assist in marketing efforts to position the hospice as a community resource.
- Act as the primary contact for ALFs, PCHs, and LTACs, maintaining relationships through regular visits.
- Perform additional duties as assigned by the Director of Nursing and Compliance Officer.
Physical and Mental Requirements:
The physical demands of this position require the ability to sit, use hands for various tasks, reach, and communicate effectively. Occasional lifting of up to 50 pounds may be necessary. The work environment may expose the employee to disagreeable conditions, including odors and physical contact with patients.
Mental resilience is essential, as the role involves working independently, managing stress, and demonstrating emotional stability.
Transportation: A valid driver's license, auto liability insurance, and reliable transportation are required.
