Job Summary

The RN for the Command Center is responsible for executing all aspects of bed management and transfer center operations in a safe and timely manner, in accordance with standard operating procedures and regulatory requirements. This role is inclusive of patient access, placement, transfers, and bed management. The successful candidate will utilize clinical knowledge, critical thinking, and communication skills to support the Command Center in providing a reliable, high-quality service that allows referrals, transfers, and other healthcare settings to be accepted in an efficient and timely manner.

Key Responsibilities
  • Assess, direct, and coordinate transfers and admissions from physician offices, hospitals, and other healthcare settings to the most appropriate accepting service, facility, and level of care.
  • Facilitate patient placement by assessing patient diagnosis, acuity, and location, applicable risk factors, and unit criteria/bed availability.
  • Collaborate with facility departments, medical staff, nursing staff, and leadership to promote appropriate, timely patient placement, ensuring patient care is not delayed.
  • Collaborate with facility staff to identify and communicate bed capacity issues.
  • Leverage capacity dashboards and other real-time indicators to predict future patient placement pressure points and participate in the execution of surge-capacity protocols.
  • Initiate interventions, within scope of practice, to accelerate patient flow or other mitigating barriers to optimal throughput.
  • Assist with all aspects of patient logistics, including pre-registration, transportation, and bed planning.
  • Collect unit-specific data and information needed to access and place patients appropriately.
  • Adhere to regulatory compliance with all Command Center operations, including Stark, EMTALA, and any other pertinent to Call Center activities.
  • Maintain current knowledge of regulatory requirements impacting operations of the Command Center.
  • Provide timely and accurate transfer and admission documentation.
  • Work collaboratively with providers and key stakeholders in the development and maintenance of transfer and admission pathways for patients.
  • Delegate tasks as needed to the Transfer Center Tech, Patient Placement Specialist, and EMS Liaison.
  • Promote the mission, vision, and values of Intermountain Health and abide by service behavior standards.
  • Perform other duties as assigned.
Requirements
  • Bachelor's degree in Nursing (BSN)
  • Current license as a Registered Nurse in the state of practice
  • Current American Heart Association Basic Life Support for Healthcare Providers (BLS-HCP) certification upon hire or obtained within 60 days of hire with prior approval
  • Minimum 5 years RN experience in either ED, ICU, and/or 5 years acute care inpatient nursing
Preferred Qualifications
  • ACLS, PALS
  • A specialty certification (CCRN, PCCN, MSRN, or CEN)
  • Critical Care or ED background
  • Hospital Supervisor, Charge Nurse, or previous transfer center experience
  • Experience with hospital throughput or capacity management
  • Experience with Command Center operations, knowledge of Epic and/or TeleTracking
Physical Requirements
  • Ongoing need for employee to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
  • Frequent interactions with providers, colleagues, customers, patients/clients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy.
