Job Summary

We are seeking a highly skilled and experienced Command Center Registered Nurse PRN to join our team at SCL Health. As a key member of our Command Center, you will be responsible for executing all aspects of bed management and transfer center operations in a safe and timely manner, in accordance with standard operating procedures and regulatory requirements.

Key Responsibilities
  • Assess, direct, and coordinate transfers and admissions from physician offices, hospitals, and other healthcare settings to the most appropriate accepting service, facility, and level of care.
  • Facilitate patient placement by assessing patient diagnosis, acuity, and location, applicable risk factors, and unit criteria/bed availability.
  • Collaborate with facility departments, medical staff, nursing staff, and leadership to promote appropriate, timely patient placement, and identify and communicate bed capacity issues.
  • Leverage capacity dashboards and other real-time indicators to predict future patient placement pressure points and participate in the execution of surge-capacity protocols.
  • Assist with all aspects of patient logistics, including pre-registration, transportation, and bed planning, and collect unit-specific data and information needed to access and place patients appropriately.
  • Adhere to regulatory compliance with all Command Center operations, including Stark, EMTALA, and any other pertinent requirements.
  • Provide timely and accurate transfer and admission documentation.
  • Work collaboratively with providers and key stakeholders to develop and maintain transfer and admission pathways for patients.
  • Delegate tasks as needed to the Transfer Center Tech and EMS Liaison.
  • Promote the mission, vision, and values of SCL Health, and abide by service behavior standards.
Requirements
  • Current license as a Registered Nurse in the state of practice is required.
  • Current American Heart Association Basic Life Support for Healthcare Providers (BLS-HCP) certification upon hire or obtained within 60 days of hire with prior approval.
  • Minimum 5 years of RN experience in either ED, ICU, and/or 5 years of acute care inpatient nursing.
  • Preferred: Critical Care or ED background, Hospital Supervisor, Charge Nurse, or previous transfer center experience, experience with hospital throughput or capacity management, and experience with Command Center operations, knowledge of Epic.
Preferred Qualifications
  • ACLS certification.
  • A specialty certification (CCRN, PCCN, MSRN, or CEN).
Physical Requirements

Regular attendance to perform work on site during regularly scheduled business hours or scheduled shifts is required. Ability to work nights and/or weekends is required for identified positions.
