Job Title: Care Facilitator - Centralized CareJob Summary:

Sparrow Health System is seeking a highly skilled Care Facilitator to join our team in a centralized care role. As a Care Facilitator, you will play a vital part in our population health strategy, working closely with our ambulatory care management team to provide comprehensive care coordination services to our patients.

Key Responsibilities:
  • Provide self-management education and support to patients and families
  • Educate and support care team members in patient self-management support concepts and techniques
  • Work with patients to set self-management goals and develop action plans
  • Assist patients and families in recognizing and prioritizing problems that need to be addressed
  • Provide support to help patients and families cope with the emotional impact of their illness
  • Problem solve with patients and families to maximize their ability to follow preventive, remedial, and rehabilitative recommendations
  • Provide individual or group counseling/coaching as appropriate
  • Educate and support office staff in providing care coordination
  • Develop and maintain effective professional working relationships with patients, families, physicians/providers, and healthcare team members
  • Coordinate care with payer case managers for patients with complex or catastrophic conditions
  • Develop, recommend, and implement plans to address patient problems and needs
  • Provide chronic disease education as needed
  • Provide consultation services for other department staff
  • Assist in developing processes to track care coordination activities
  • Assist in developing written procedures and guidelines on care coordination processes and train care team members
  • Facilitate follow-up with patients discharged from the hospital or Emergency Department as needed
Requirements:
  • Registered Nurse with a current Michigan license
  • Minimum of three years acute care clinical experience as a registered nurse
  • Minimum one year case management experience
  • Bachelor's Degree in Nursing or healthcare-related field
  • Demonstrates excellent clinical knowledge, skills, and judgment
  • Demonstrates excellent communication skills, including emotional intelligence, relationship building, negotiation, conflict resolution, persuasion, marketing, and patient advocacy
  • Demonstrates comprehensive case management assessment skills, with the ability to proactively and creatively problem solve
  • Demonstrates the ability to prioritize, organize, handle many tasks simultaneously, work autonomously, and manage time
  • Demonstrates the ability to work in stressful situations, manage conflict, and assume a leadership role
Preferred Qualifications:
  • National case management certification
  • Graduate degree in nursing or related healthcare field
  • Knowledge of case management, including healthcare finance, hospital and community resources, discharge planning, utilization review, utilization management, ethical case management principles, and evidence-based practice concepts
  • Knowledge and demonstrated ability to collect, analyze, and utilize data for process improvement
  • Experience with an EMR
