Clinical Care Coordinator

We are seeking a skilled Clinical Care Coordinator to join our team at McLaren Health Care. As a key member of our healthcare team, you will provide direct patient care and comprehensive coordination of care for patients and their families/caregivers.
Key Responsibilities:
- Eliminate barriers to timely care and facilitate flow through the system
- Provide a full range of nursing services, including assessment, treatment planning, and therapeutic interventions
- Oversee the planning, scheduling, and implementation of day-to-day clinical activities and procedures
- Collect and report data from patients on barriers, healthcare, medications, and prior treatments
- Facilitate all stages of the referral process to outside resources and care coordination as needed
Requirements:
- Nursing degree from an accredited educational institution
- State licensure as a registered nurse
- Bachelor's degree in nursing preferred
- One year prior work experience in a clinical area preferred
- BLS certification
