Position Overview

The Patient Services Coordinator II is integral to the effective delivery of healthcare services within the cardiology department. This role acts as a vital link in patient care operations, ensuring that high-quality service is maintained throughout patient interactions. The Coordinator is responsible for educating patients regarding their treatment plans and facilitating communication with Primary Care Providers and diagnostic testing facilities. The focus is on prioritizing tasks that align with the needs of patients, their families, and caregivers.

Key Responsibilities

1. Collaborates closely with Primary Care Providers to streamline patient care plans, engaging in daily discussions and monthly case reviews, managing appointments, and coordinating essential information and ongoing care needs.

2. Works alongside clinical staff to address and resolve challenges that may hinder patient care progress.

3. Oversees and updates a task management dashboard, ensuring timely reminders and alerts for daily responsibilities such as visit status, follow-up appointments, and authorizations for medical reviews.

4. Engages with insurance providers for patient pre-certifications and consults with management regarding patient needs or escalated concerns.

5. Provides necessary front-end support, documenting telephone interactions for appropriate routing to healthcare providers.

6. Acts as a liaison among physicians, patients, and clinical teams to connect patients with relevant internal and external resources.

7. Suggests improvements to processes and services aimed at enhancing patient satisfaction and addressing concerns.

8. Delivers informed and considerate responses to patients, guiding them through their healthcare choices.

9. Maintains a solid understanding of medical terminology and practices relevant to the healthcare environment through ongoing education.

10. Performs additional duties as required.

Qualifications

A minimum of 4 years of experience in healthcare or customer service roles is required.
