POSITION SUMMARY:


The Outpatient Registered Nurse (RN) CAP 2 serves as a pivotal role within the Clinical Advancement Program (CAP), catering to newly hired nurses who fulfill the RN CAP 2 criteria or those advancing from RN CAP 1.

This role is integral to ensuring the delivery and coordination of high-quality clinical care, encompassing assessment, planning, intervention, and evaluation for a designated patient cohort.

Responsibilities may involve delegating suitable tasks to direct patient care personnel, including but not limited to RNs, LVN/LPNs, and Patient Care Technicians.

As a vital member of the End Stage Kidney Disease (ESKD) healthcare team, the RN participates in decision-making processes, educational initiatives, leadership roles, and quality enhancement activities aimed at improving patient care outcomes and operational efficiency.


KEY RESPONSIBILITIES:


All tasks and responsibilities must be executed in alignment with Fresenius Kidney Care policies, nursing standards, and applicable state and federal regulations.


Under the supervision of the Team Leader, the RN will perform essential functions with guidance from Clinical Educators, Preceptors, or in collaboration with fellow Registered Nurses.


Conducts systematic and ongoing collection and analysis of patient data before, during, and after hemodialysis treatments for assigned patients, documenting findings in the medical record, and making necessary adjustments to treatment plans while notifying the appropriate team members as required.

Assesses and documents the educational needs of patients and their families to provide both initial and ongoing education.


Collaborates with direct and ancillary patient care staff to ensure the safe and effective delivery of hemodialysis therapy to assigned patients.

Administers prescribed medications in accordance with established protocols and documents the medical rationale and effectiveness.

Initiates or assists in emergency response measures as necessary.


Acts as a resource, leader, coach, mentor, and role model for both new and existing staff, exemplifying appropriate behavior, work ethics, and attitudes towards patients, colleagues, supervisors, and the organization.

Ensures proper laboratory collection, processing, and shipping procedures are adhered to and reschedules any missed or inadequate laboratory collections.


Identifies expected outcomes, documents, and updates nursing assessments and care plans for assigned patients in collaboration with the Interdisciplinary Team.

Ensures patient awareness regarding transplant and treatment modality options.

Engages in educational and quality improvement initiatives at both the facility and area levels as directed by the Supervisor.

May serve as a Preceptor for new staff members.

Required to fulfill CAP requirements for maintenance or advancement.

Performs additional duties as assigned by the Supervisor.

PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT:

This role involves direct patient care that frequently requires heavy lifting, patient mobility assistance, and support with ambulation. Equipment aids and/or colleagues may provide assistance.

Frequent, prolonged periods of standing are required, and the employee must be capable of bending over.


Occasionally, the employee may need to move, with assistance, machines and equipment weighing up to 200 lbs., and may lift chemical and water solutions weighing up to 30 lbs. as high as 5 feet.

The work environment is typical of a healthcare facility, featuring controlled air temperature and moderate noise levels.

Exposure to infectious and contagious diseases/materials may occur.

Daily responsibilities include desk work, computer tasks, and interaction with patients, facility staff, and physicians.

This position may require travel to training sites or other facilities.


May be asked to perform essential functions of this role in other locations with similar physical demands and working conditions as described above.


SUPERVISORY RESPONSIBILITIES:
Assigned oversight of Patient Care Technicians, LVN/LPNs, and RNs as a Team Leader or designated Nurse in Charge, upon meeting all necessary qualifications:

Successful completion of all Fresenius Kidney Care education and training requirements for new hires.

A minimum of 3 months experience in chronic/acute hemodialysis as a Registered Nurse is required.

EDUCATION AND LICENSURE:
Graduate of an accredited School of Nursing.

Current state licensure appropriate for the role.

Current or successful completion of CPR BLS Certification.

Must meet practice requirements in the state of employment.

EXPERIENCE AND REQUIRED SKILLS:
Entry-level for RNs with a minimum of 2 years of experience in Nephrology Nursing.


The compensation for this position will be determined based on the successful candidate's work location and qualifications, including relevant education, work experience, skills, and competencies.


HOURLY RATE:
$62.00
