Job Title: Hospice Clinical Liaison

At Compassus, we are seeking a highly skilled and compassionate Hospice Clinical Liaison to join our team. As a key member of our organization, you will play a vital role in promoting our values of Compassion, Integrity, Excellence, Teamwork, and Innovation.

Job Summary:

The Hospice Clinical Liaison is responsible for modeling our company values and promoting our philosophy, using the 6 Pillars of Success as the foundation. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with referral sources, patients, and their families.

Key Responsibilities:
  • Attend and participate in staff meetings, educational programs, and community events as requested.
  • Communicate identified needs and potential solutions to the Supervisor.
  • Explain hospice services and benefits to patients, family members, and significant others in a clear and concise manner.
  • Obtain signatures on all required forms and provide copies to patient/responsible party.
  • Ensure all forms are completed, dated, signed, and turned in to the Admission Coordinator.
  • Establish and maintain significant professional relationships.
  • Work closely with community, hospital, and facility education representatives to meet growth and development targets and maintain market acceptance and allegiance.
Requirements:
  • Active and unencumbered Registered Nurse license, or Licensed Practical/Vocational Nurse, in state(s) of employment required.
  • Associate or Bachelor's degree in Nursing highly preferred.
  • Two (2) to three (3) years of related experience required.
  • Hospital and/or long-term care clinical experience highly preferred.
  • Experience in healthcare marketing preferred.
Physical Demands and Work Environment:

The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
