Hospice LPN Care Specialist

Responsibilities:
* Administer skilled nursing care to patients in their homes, under the supervision of a registered nurse
* Coordinate care with the interdisciplinary team, patient, and family
* Monitor patient progress and adjust care plans as needed
* Provide education and support to patients and families
* Collaborate with other healthcare professionals to ensure seamless care
Requirements:
* Graduate of an accredited school of vocational nursing
* One year experience as a Licensed Vocational Nurse in a clinical care setting required
* Home health experience preferred
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Reliable transportation
