Job Summary

We are seeking a skilled and compassionate Clinical Services Coordinator to join our team at Christian Living Communities. As a Clinical Services Coordinator, you will provide high-quality clinical services to our residents, including skilled nursing, assisted living, and other care needs.

Key Responsibilities
  • Provide daily direction and assistance with residents' activities of daily living.
  • Become familiar with residents' preferences to personalize their care.
  • Assist residents with social, personal, and health care needs as required.
  • Ensure proper application of safety, infection control, and risk management procedures.
  • Make decisions in emergency situations and notify relevant personnel.
  • Communicate regularly with nursing supervisors regarding residents' status and condition.
  • Accurately document and report discrepancies to appropriate personnel.
Requirements
  • High School Diploma or equivalent.
  • Certified Nursing Assistant License-active and in good standing.
  • Ability to work well with different teams in different Communities.
  • 3-6 months experience working with senior adults preferred.
  • Excellent customer service skills.
Working Conditions
  • Position requires strenuous physical work, including heavy lifting and pushing/pulling objects up to 100 pounds occasionally and/or up to 50 pounds frequently.
  • Exposure to blood borne pathogens, body excretions, and communicable diseases possible.
  • Frequent interruptions and interactions with residents, family members, and visitors.
