Optimize Patient Recovery Journeys

As a Skilled Inpatient Care Coordinator, you'll play a vital role in optimizing patients' recovery journeys.

Key Responsibilities:
  • Complete weekly functional assessments and engage the post-acute care (PAC) inter-disciplinary care team to coordinate discharge planning.
  • Engage patients and families to share information and facilitate informed decisions.
  • Ensure efficient, smooth, and prompt transitions of care by serving as the link between patients and the appropriate healthcare personnel.
Requirements:
  • Active, unrestricted registered clinical license in Colorado - Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist.
  • 5+ years of clinical experience.
  • Reside within or near the county listed on the job description.
Preferred Qualifications:
  • Experience working with the geriatric population.
  • Patient education background, rehabilitation, and/or home health nursing experience.
  • Proficient with Microsoft Office applications including Outlook, Excel, and PowerPoint.
Work Conditions and Physical Requirements:
  • Ability to establish a home office workspace.
  • Ability to manipulate a laptop computer between office and site settings.
  • Ability to communicate with clients and team members using a cellular phone or comparable communication device.
