As a Care Manager, RN with Inland Empire Health Plan, you will play a vital role in providing high-quality care management to our members. Under the supervision of the Enhanced Care Management Department Leadership, you will work effectively with the Enhanced Care Management team to promote health behaviors, coordinate care, and facilitate seamless transitions of care.

This position requires a strong understanding of self-management philosophies and practices, especially as they relate to chronic medical conditions. You will be responsible for building and maintaining positive working relationships with providers, engaging with members in a collaborative manner, and promoting a collaborative and effective working environment within the Enhanced Care Management team.

Key responsibilities include:

  • Assimilating assessment information into individualized care plans (ICPs)
  • Communicating ICPs with members, approved family or caregivers, and other members of the care team
  • Leading inter/transdisciplinary care team meetings to share information, update, and inform care plans
  • Coordinating with internal and external health partners to support members' comprehensive care needs
  • Participating and leading care transition plan responsibilities

Requirements include:

  • Three (3) or more years of care management experience in a healthcare delivery setting
  • Associate's degree in Nursing from an accredited institution
  • Experience in a healthcare organization or experience in managed care setting preferred
  • Minimum of one (1) year clinical experience in an acute care facility, skilled nursing facility, home health, or clinic setting preferred
  • Bilingual (English/Spanish) preferred
  • Understanding of and sensitivity to multi-cultural community
