Job Summary

We are seeking a highly skilled and experienced Director of Case Management and Discharge Planning to join our team at Samaritan Medical Center. As a key member of our leadership team, you will be responsible for leading the planning, implementation, and maintenance of all case management and discharge planning processes across our organization.

Key Responsibilities
  • Oversee and manage all aspects of case management, discharge planning, and inpatient denial management.
  • Ensure compliance with accreditation and regulatory standards at the local, state, and federal levels.
  • Manage departmental budgets, facilitate quarterly reviews of the PEPPER report, and oversee PRI screen completion.
  • Lead initiatives to reduce length of stay, improve care for uninsured/underinsured patients, and enhance overall resource utilization.
  • Participate in key organizational initiatives including task forces, revenue cycle work groups, and community advisory boards.
  • Foster collaborative relationships with physicians, home care agencies, social services, and hospice to ensure seamless care transitions.

The ideal candidate will demonstrate strong leadership skills, a high degree of innovation, and effective problem-solving abilities. This hands-on leader will contribute to the success of ongoing organizational initiatives and actively participate in community and departmental initiatives.

Required Qualifications
  • Bachelor's degree in a relevant field is required.
  • Active NYS RN license is required.
  • A minimum of 5 years of experience in healthcare, case management, or discharge planning.
  • Proven knowledge of accreditation standards, payer regulations, and the Family Healthcare Decision Act.
