*Hybrid* Autism Clinical Case Manager & Advocate

This unique opportunity offers flexibility and truly makes a difference in our clients’ lives, as we support individuals long-term as they navigate adulthood. We call our team leaders “Clinical Case Managers / Personal Advocates,” and the key responsibilities are:

Primary responsibilities:

  • Build rapport and trusting relationships with a caseload of approximately 12-15 adults

  • Hold regularly scheduled 1:1 check-ins with clients via video call or phone and offer occasional meetings in person with clients

  • Assess, identify, and prioritize clients' needs to inform the development and implementation of a personalized support plan

  • Lead direct support professional colleagues (“support partners”) in their work supporting shared clients; assist in their professional and personal development

  • Coordinate community-based visits by support partners and facilitate collaboration with clients' support networks

  • Monitor clients' health, safety, and well-being at home, work, and in the community

  • Document progress and update support plans

  • When indicated and with support of clinical leadership, connect clients with crisis management resources

  • Support community social events, client projects, resource sharing, and/or program development tasks as needed and desired


Requirements:

  • Valid driver's license, clean record, and reliable vehicle

  • Master’s degree or higher and licensure in occupational therapy, social work, mental health counseling, or related clinical field

  • Professional, academic, and/or personal experience working with neurodiverse adults preferred (or similar/transferable experience)

  • Competence in supporting clients across various areas of need

  • Interpersonal, organizational, and time management skills

  • Ability to work independently and as part of a team

  • Willingness to participate in ongoing professional development
