Job Overview
The Licensed Practical Nurse (LPN) plays a crucial role in delivering safe and effective patient care under the guidance of a Registered Nurse (RN). The LPN is involved in monitoring, observing, and reporting patient responses in accordance with the established care plan.
Work Schedule
We are seeking full-time candidates for day or night shifts, with hours from 7:00 am to 7:00 pm or 7:00 pm to 7:00 am, totaling 72 hours per pay period.
Unit Description
The Acute Rehabilitation unit is a specialized 10-bed facility that holds CARF accreditation. Our focus is primarily on neuro-rehabilitation, including care for patients recovering from strokes, brain injuries, and multi-trauma incidents. We foster a collaborative environment that emphasizes teamwork among healthcare professionals to support our patients' goals of achieving independence and returning home. The average length of stay for our patients is approximately two weeks, with staffing consisting of two nurses and one technician during the day and two nurses at night.

Key Responsibilities
Clinical Excellence
- Collaborate with the healthcare team to deliver high-quality patient care and achieve optimal outcomes.
- Participate in the admission assessment process by accurately collecting and documenting patient data.
- Assist in identifying appropriate interventions to maintain patient comfort and safety.
- Administer medications and perform nursing tasks within the scope of practice.
- Communicate effectively with patients and families to ensure safe transitions in care.
Effective Communication
- Notify the assigned RN of any changes in patient conditions promptly.
- Engage with the healthcare team to discuss progress towards care goals.
- Document patient information accurately and in real-time in the electronic medical record.
Patient Education
- Provide comprehensive education to patients and families throughout their care journey.
- Assess learning needs and tailor teaching strategies to individual patient requirements.
Qualifications
Minimum Requirements:
- Completion of an accredited LPN program.
- Current, valid LPN licensure in the applicable state.
- Basic Life Support (BLS) certification.
Preferred Qualifications:
- Experience in a rehabilitation setting.
- IV Therapy certification.
Skills and Abilities:
- Proficient in English communication, both written and verbal.
- Demonstrated ability to perform basic nursing procedures and operate necessary equipment.
