Job Description

Job Title: Senior Case Manager

We are seeking a highly skilled Senior Case Manager to join our team at Westhab Inc. The ideal candidate will have a strong background in case management, excellent communication skills, and the ability to work collaboratively with other departments.

Key Responsibilities:

  • Manage intake and develop comprehensive assessments for new participants.
  • Develop systems to ensure quality and efficiency of casework services.
  • Ensure staff identifies client needs, provides referrals, and follows up with resources to promote successful achievement of service plan goals.
  • Document assessments, service plans, and client interactions.
  • Review case management documentation and data collection tools.
  • Supervise service staff and ensure clear, concise, and professional interventions.
  • Build collaboration with community service providers.
  • Maintain documentation and submit reports in a timely manner.
  • Provide crisis intervention and stabilization as needed.
  • Identify program needs and make recommendations.

Requirements:

  • LMSW with 3 years of relevant experience and supervisory experience.
  • Fluency in Spanish is highly desirable.
  • Ability to work some evenings and occasional weekends as needed.
  • Excellent writing ability and strong assessment, direct practice, counseling, supervision, community outreach, group leadership, and program development experience.
  • Advanced knowledge of MS Word, Excel, and database management skills.
  • Must have own car and valid NYS driver's license.
