Job Title: Senior Case Manager

Samaritan Daytop Village, Inc. is seeking a highly skilled and experienced Senior Case Manager to join our team.

About the Role

As a Senior Case Manager, you will be responsible for providing supervision and guidance to Case Managers, ensuring the delivery of high-quality services to clients. You will work closely with clients to develop and implement Independent Living Plans, locate and utilize community resources, and facilitate re-socialization processes.

Key Responsibilities
  • Supervise and guide Case Managers to ensure the delivery of high-quality services
  • Develop and implement Independent Living Plans for clients
  • Locate and utilize community resources to support clients
  • Facilitate re-socialization processes for clients
  • Provide individual counseling and group facilitation
  • Administer behavioral interventions as needed
  • Participate in assessment screenings and treatment planning
Requirements

To be successful in this role, you will need:

  • A high school diploma or equivalent with 5 years of experience in case management or a related field
  • OR an associate's degree in human services or a related field with 2-3 years of experience in case management or a related field
  • OR a bachelor's degree in human services with 1-2 years of experience in case management or a related field
  • Knowledge of CARES is a plus
  • Excellent time management and communication skills
  • Experience working with diverse populations
  • Strong writing skills and proficiency in Microsoft Office Suite and EMR
Preferred Qualifications

We prefer candidates with:

  • Ability to participate in CRP-related trainings
  • Bilingual Spanish
