We are seeking a highly skilled and compassionate Patient Care Manager to join our team at Extendicare Home Health of West Tennessee. As a Patient Care Manager, you will play a vital role in the supervision and coordination of clinical services, ensuring that patients receive high-quality care in the comfort of their own homes.
Key Responsibilities:
- Supervise and coordinate clinical services to ensure that patients receive high-quality care in accordance with physician orders and applicable laws and regulations.
- Provide and direct provisions of nursing care to patients in their homes, as prescribed by the physician and in compliance with agency policies.
- Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and plots start of care visits.
- Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate.
Requirements:
- Current RN licensure in the state of practice
- Current CPR certification required
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
