Job Title: RN Care Management Supervisor

Franciscan Health is seeking a skilled RN Care Management Supervisor to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional care to our patients and their families.

Responsibilities:
  • Participate in people management activities for direct team members, including performance evaluations, disciplinary actions, and interviews.
  • Contribute to performance improvement activities to enhance patient care and outcomes.
  • Provide education to beneficiaries, support systems, and healthcare teams on disease management and evidence-based practice guidelines.
  • Document and communicate changes in health status to key individuals, including primary care physicians and clinicians.
  • Perform interventions and document to meet the needs of beneficiaries, their families, and healthcare teams based on scientific principles and clinical expertise.
  • Utilize motivational and health-coaching techniques to engage beneficiaries and support systems.
  • Assess, intervene, and interview beneficiaries as needed when consultation is requested by providers, Clinical Case Coordinators, or other clinicians.
  • Determine the effectiveness of care plans through beneficiary assessment and document healthcare outcomes.
  • Provide or facilitate education to facilitate the achievement of health goals using a beneficiary and family-centered approach.
  • Reassess and modify care plans as necessary to achieve health goals.
  • Understand and apply federal and state regulations effectively, demonstrating knowledge of ACO/ICM organizational policies.
  • Maintain a seamless and coordinated acute/post-acute care continuum for ACO patients discharged from Franciscan Health facilities systemwide.
Qualifications:
  • Bachelor's Degree in Nursing required.
  • Active Indiana RN license required.
  • 5 years of hospital/managed care experience required.
  • 5 years of current nursing experience required.
  • 2 years of case management/coordination experience required.
  • Basic Life Support Program (BLS) & Cardiopulmonary Resuscitation (CPR) - American Heart Association required.
  • Certified Case Manager (CCM) or Commission for Case Manager Certification (CCMC) strongly preferred.
  • Prior leadership experience preferred.
  • Experience with hospital information systems (e.g., order entry, results reporting) preferred.

Travel is Required: Up to 50%
