About the Role:

Pathways to Housing DC is seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Clinical Health Specialist. In this role, you will provide primary healthcare to adults with serious mental illness, promoting wellness, recovery, and rehabilitation.

Key Responsibilities:

  • Conduct home visits to assist consumers with medication management and health education
  • Collaborate with other healthcare providers and make appropriate referrals
  • Teach health promotion to consumers and staff
  • Advocate for consumers in accessing and navigating the healthcare system
  • Collaborate with staff to coordinate prescriptions and orders, and to oversee the overall health of consumers
  • Engage qualified consumers in medical services, both on-site and in the community
  • Participate in facilitating appropriate hospitalizations (medical and psychiatric), crisis interventions, and referrals
  • Communicate with hospital staff to ensure optimal treatment and discharge planning

Requirements:

  • Certification by the American Nurses Credentialing Center
  • Experience or training in community preferred
  • COVID vaccine required
