POSITION OVERVIEW

The Care Navigator plays a vital role within an Integrated Care team that encompasses the Primary Care Provider (PCP), Behavioral Health Clinician (BHC), and psychiatric provider. This position is tasked with coordinating and facilitating Mental Health services within the clinic, as well as managing referrals to necessary services both within and outside the clinic. The Care Navigator may deliver evidence-based interventions to patients or collaborate with other Mental Health professionals when appropriate. This role will report to the Manager of Behavioral Health/Primary Care Integration or their designated representative.

EMPLOYMENT STATUS: Full Time (36 hours)

WORK LOCATION: Clifton Springs Hospital

DEPARTMENT: Inpatient Psychiatric

SCHEDULE: Days, 7AM-7:30PM, every other weekend.

QUALIFICATIONS
  • A minimum of 2 years of experience in a Human Services or medical-related field, with direct client/patient interaction required.
KEY RESPONSIBILITIES
  • Facilitate and coordinate behavioral health services in collaboration with the patient, their Primary Care Provider (PCP), BHC, psychiatric provider, and other Integrated Care team members.
  • Educate patients about prevalent mental health and substance use disorders, along with available treatment options.
  • Monitor patients (in-person or via telephone) for any changes in clinical symptoms and treatment side effects or complications.
  • Provide or arrange referrals to mental health clinics/substance use disorder treatment programs as clinically warranted.
  • Engage in regularly scheduled Integrated Care team meetings and communicate treatment recommendations to team members and external collaborators as necessary.
  • Encourage patient engagement and ensure follow-up in care.
  • Handle confidential information with care and sensitivity.

Associate's Degree (Required)

S - Sedentary Work - Involves exerting up to 10 pounds of force occasionally. Sedentary work typically involves sitting most of the time, with occasional walking or standing.

The stated base pay range reflects a good faith representation of potential base pay for a successful full-time candidate. It may be adjusted in the future and is eligible for additional pay components.
