    Middletown Park Rehabilitation & Healthcare is a 230-bed 5 STAR rated skilled nursing facility dedicated to being a premier provider in short-term rehabilitation, subacute, & long-term healthcare services and trusted by families to care for their loved ones for over 30 years.

Responsibilities:

  • Provide daily support to unit leadership and respond appropriately and timely to resident and family needs within the LPN scope of practice.
  • Utilize nursing processes, judgment, and skills to prioritize and deliver quality direct resident care, ensuring safety and quality in all aspects of care.
  • Administer standard nursing treatments and medications while observing residents for desired or unwanted effects.
  • Communicate and collaborate with care team members to revise and update the Nursing Care Plan based on pertinent findings, including lab/test results and incidents.
  • Monitor and document vital signs, treatments administered, and residents' responses.
  • Prepare, organize, oversee, monitor, and evaluate the delivery of care of resident CNA assignments and activities, considering the needs and competency of personnel.

Requirements:

  • Graduate of an approved school of Nursing.
  • Current license in good standing in New York as an LPN.
  • Dedicated to providing extraordinary, clinically sound, and compassionate care.
  • Possess a spirit of cooperation, caring, and enthusiasm to create an atmosphere conducive to rehabilitation and optimal resident health.
  • Genuinely interested in helping ill, injured, disabled, elderly, and sometimes challenging residents.
  • Excellent communication, customer service, and organizational skills.
  • Ability to work well under pressure and harmoniously with all staff.
