We are seeking a compassionate and dedicated End-of-Life Care Professional to join our team at Hospice of the West. In this role, you will play a vital part in providing patient-centered care to individuals in need.

Responsibilities
  • Deliver personalized care to patients, prioritizing their comfort and well-being
  • Support patients with various personal care needs, maintaining dignity and respect
  • Provide exceptional customer service, exceeding patient satisfaction expectations
  • Collaborate with families, offering guidance and support during challenging times
  • Maintain accurate documentation, adhering to agency and regulatory standards
Requirements
  • High School Diploma or GED required
  • Current unencumbered CNA certification in the state of practice
  • Minimum one year experience as a CNA in an acute care or long-term care setting
  • Possess current CPR certification if state-mandated
