Job Summary
We are seeking a compassionate and skilled Nurse Practitioner to provide high-quality medical care to seniors and individuals with complex needs. As part of our team, you will work in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes.

About the Role
This is a unique opportunity to make a meaningful difference in people's lives while growing your medical skills and expertise. You will be responsible for providing evidence-based medical care, examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.

Key Responsibilities
• Performs comprehensive assessments and develops individualized plans of care
• Orders and interprets laboratory and diagnostic testing as needed
• Collaborates with caregivers and other healthcare professionals to ensure seamless patient care
• Provides referrals to other healthcare practitioners when necessary
• Ensures accurate and up-to-date patient medical records

Requirements
• Active state Nurse Practitioners license
• Current Nurse Practitioner National Board Certification
• Active state DEA licensure
• Master's degree in Nursing (MSN)
• Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
• Strong interpersonal communication skills – verbal and written
• Excellent time management skills with accountability for practitioner's daily workflow

Benefits
