Job Title: Family Team Conference Facilitator

Harlem Dowling-West Side Center for Children and Family Services is seeking a highly skilled and dedicated Family Team Conference Facilitator to join our team. As a key member of our family support programs, you will play a vital role in promoting effective permanency planning and family engagement.

Key Responsibilities:
  • Conduct Family Team Conferences (FTCs) for family support programs, ensuring effective permanency planning and family engagement.
  • Collaborate with families, social workers, and other stakeholders to develop and implement individualized plans.
  • Provide education and support to families on permanency planning, child welfare, and related topics.
  • Develop and maintain relationships with community resources and services to support families.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements:
  • Minimum Bachelor's Degree with 3 years related experience, MSW preferred.
  • Minimum 1-year experience conducting FTCs.
  • Bi-lingual in Spanish required.
  • Computer Literate with proficiency in Excel, Word, and CONNECTIONS.
  • Ability to communicate effectively both oral and in writing.
  • Well organized, able to meet deadlines and manage successfully multiple assignments.
  • Extensive knowledge of ACS and OCFS systems.
  • Extensive knowledge of Foster Care Regulations related to FTCs ICPCs and SIJS.
