Position Overview:


The Hospice Licensed Practical Nurse (LPN) will provide direct patient care, utilizing a variety of skills and coordinating necessary services.
Responsibilities include:

  • Completing required documentation in accordance with policies, procedures, and regulations.
  • Implementing the care plan tailored to the patient's diagnosis and condition as developed by the Registered Nurse.
  • Coordinating hospice services with the Registered Nurse, Patient Care Coordinator, and other personnel to enhance patient care.
  • Transcribing and accepting physician's orders as directed.
  • Forwarding all clinical, progress, and summary notes to the Hospice Registered Nurse for care coordination.
  • Participating in Interdisciplinary Team (IDT) meetings.

Qualifications:


The ideal candidate will possess:

  • Successful completion of nursing education from an accredited nursing program.
  • Current LPN licensure in the respective state and a minimum of one year of clinical nursing experience, preferably in an acute care medical/surgical environment.
  • Ability to independently organize tasks and deliver effective healthcare services to hospice patients.
  • Capability to ensure appropriate patient care under the supervision of the Hospice Registered Nurse, with thorough, accurate, and timely documentation.
  • Strong communication skills, both oral and written.
  • Current CPR certification.
