">Role Description
">This is an exciting opportunity for individuals who are passionate about delivering exceptional care and making a positive impact on the lives of others. As a Disability and Aged Care Practitioner, you will be responsible for providing high-quality in-home care and support for our clients, promoting their independence and well-being, and enhancing their quality of life.
">Responsibilities
">- Providing personal care, including bathing, dressing, grooming, and other hygiene tasks
- Assisting with mobility, including transferring from bed to wheelchair, walking, and positioning
- Prompting prescribed medication administration and monitoring health conditions as directed
- Providing emotional support, engaging in meaningful conversation, and participating in activities to enhance the client's quality of life
- Assisting with light housekeeping, meal preparation, and laundry
- Maintaining accurate records of care provided and reporting any changes in the client's condition to the healthcare team
Requirements
">To be successful in this role, you will need to possess a Certificate III or IV in Aged Care, Community Care, or Disability Services, along with a current National Police Clearance, First Aid and CPR certificate, Working With Children Card, and relevant immunisations for working in WA health. Additionally, a C-Class driver's licence and own roadworthy, registered vehicle with comprehensive car insurance are required.

">We offer a competitive salary range of $65,000 - $80,000 per annum, depending on experience, plus benefits such as flexible work arrangements, ongoing training and professional development opportunities, and a supportive team environment.
