Care Coordination Professional

About the Role:
In this critical role, you will be responsible for developing and maintaining good working relationships with health care providers, community based agencies, and other departments/disciplines. You will provide timely and appropriate client assessments, care planning, and progress notes as part of care coordination. Your expertise in case management will enable you to identify appropriate interventions within program/department parameters, protocols, and procedures.

Requirements:
* Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field* Current applicable state(s) license as a Licensed Social Worker preferred* Valid driver's license and reliable vehicle required* With a bachelor's degree: Two years of experience in a community health setting, including one year of case management experience required* In lieu of bachelor's degree: Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience* Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred
Estimated Salary: $58,000 - $68,000 per annum
