Job Summary:

The Care Coordinator will be responsible for completing intake forms, preparing change of address records, and issuing service discontinuance orders. They will also confer with customers to provide information about services, maintain records of customer interactions, and communicate with family members to assist them in understanding and supporting consumers.


Responsibilities:

  • Complete intake forms using an iPad and prepare change of address records, and issue service discontinuance orders.
  • Confer with customers in person and by telephone to provide information about services.
  • Maintain records of customer interactions and transactions, recording details of inquiries, complaints, and comments, as well as actions taken.
  • Communicate with family members to assist them in understanding, dealing with, and supporting consumers with respect to Self Determination process.
  • Identify opportunities to add to or improve our service offerings.
  • Maintain confidentiality of records relating to consumers.
  • Uphold mission and values.
  • Perform other duties as assigned.


Requirements:

  • Degree in Business, Human Services, Marketing/Communication, or comparable experience and education.
  • Will consider candidates with experience relevant to position.
