Pharmacist - Medication Expert

We are seeking a highly skilled Clinical Pharmacist to join our team at PeaceHealth. As a Clinical Pharmacist, you will play a critical role in ensuring the optimal use of medications to achieve specific outcomes that improve a patient's quality of life.
Key Responsibilities- Review medication orders for appropriateness and accuracy, including contraindications, allergies, dose, route, frequency, duplicate therapy, and formulary status.
- Interpret, clarify, and rectify physician medication and IV orders, including the medication reconciliation process.
- Interpret, clarify, and rectify prescriptions and select medications for outpatient dispensing.
- Monitor all patients on specified target drugs and initiate pharmacokinetic dosing protocols.
- Evaluate drug therapy for appropriateness and make drug therapy recommendations to clinicians.
- Prescribe drug therapy per collaborative drug therapy management agreements/Medical Executive Committee-approved protocols.
- Dispense the final product for administration and perform patient counseling.
- Detect, monitor, and report adverse drug events and medication errors (EIR).
- Supervise and train pharmacy technicians to ensure accurate order entry, preparation, and proper technique.
- Supervise and train pharmacy interns and externs. May serve as preceptor for the intern or extern as defined by the State Board of Pharmacy.
- Participate in the development of drug protocols and clinical pathways.
- Perform drug usage evaluations on selected drugs and report results to the Pharmacy & Therapeutics Committee. Complete quality assurance auditing as required for prescriptive authority.
- Perform other duties as assigned.
- Bachelor's Degree in Pharmacy or Doctor of Pharmacy from an accredited school.
- Demonstrated basic pharmacy clinical skills.
- Demonstrated customer service skills.
- Pharmacist - Washington currently licensed or able to obtain license.
- Basic Life Support certification preferred.
- Advanced Cardiac Life Support certification preferred.
- Pediatric Advanced Life Support certification preferred.
- Basic computer and keyboarding skills, including MS Office applications.
- Ability to identify medications and read graduation marks on syringes and flasks.
- Strong planning, prioritizing, and organizational skills.
- Excellent communication skills, both verbal and written.
- Ability to think and act independently.
- Strong mathematical and analytical skills.
This position requires the ability to move around the area with occasional sitting. Occasional squatting, kneeling, and bending are also required. Fine motor skills are necessary to grasp and perform precise procedures frequently. Frequent reaching is also required.
Lifting occasionally up to 30 pounds and pushing/pulling occasionally up to 14 pounds of force are also required.
Exposure to biohazard, body fluids, and airborne particles is also a requirement of this position.
Must be able to complete tasks in a noisy environment.
Vision and hearing within normal limits are required, as well as the ability to communicate and exchange accurate information.
