Job Summary:

PeaceHealth is seeking a skilled Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for ensuring the optimal use of medications to achieve specific outcomes that improve a patient's quality of life.

Responsibilities:

Reviews medication orders for appropriateness and accuracy, including contraindications, allergies, dose, route, frequency, duplicate therapy, and formulary status.

Interprets, clarifies, and rectifies physician medication and IV orders, including the medication reconciliation process.

Interprets, clarifies, and rectifies prescriptions and selects medications for outpatient dispensing.

Monitors all patients on specified target drugs and initiates pharmacokinetic dosing protocols.

Evaluates drug therapy for appropriateness and makes drug therapy recommendations to clinicians.

Prescribes drug therapy per collaborative drug therapy management agreements/Medical Executive Committee approved protocols.

Dispenses the final product for administration and performs patient counseling.

Detects, monitors, and reports adverse drug events and medication errors (EIR).

Supervises and trains pharmacy technicians to assure accurate order entry, preparation, and proper technique.

Participates in the development of drug protocols and clinical pathways.

Performs drug usage evaluations on selected drugs and reports results to the Pharmacy & Therapeutics Committee.

Requirements:

Bachelor's Degree in Pharmacy or Doctor of Pharmacy from an accredited school.

Demonstrated basic pharmacy clinical skills.

Demonstrated customer service skills.

Pharmacist - Washington currently licensed or able to obtain license.

Basic computer and keyboarding skills, including MS Office applications.

Ability to identify medications and to read graduation marks on syringes and flasks.

Strong planning, prioritizing, and organizational skills.

Excellent communication skills, both verbal and written.

Ability to think and act independently.

Strong mathematical and analytical skills.

Working Conditions:

Lifting: Occasionally up to 30 lbs.

Ability to move around area with occasional sitting.

Occasional squatting/kneeling/bending.

Fine motor skills to be able to grasp to perform precise procedures frequently.

Frequent reaching.

Environmental Conditions:

Exposure to biohazard, body fluids, and airborne particles.

Must be able to complete tasks in a noisy environment.

Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).

Ability to communicate and exchange accurate information.
