- Conduct thorough medication reviews, identify potential drug interactions, and provide personalized recommendations to patients and healthcare providers.
- Develop and implement patient-centered care plans that address individual needs and health goals.
- Collaborate with healthcare teams to provide comprehensive care services, including immunizations, diagnostic testing, and disease state management.
- Stay up-to-date with the latest clinical research, guidelines, and best practices to provide evidence-based care and promote patient safety.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care and pharmacy operations.
Requirements:
- Bachelor's degree in Pharmacy or Pharmacist degree from an accredited institution.
- Current pharmacist licensure in the states within the district.
- At least 1 year of experience as a pharmacist in a retail setting, including prescription filling and verification, records and legal compliance, pharmacy operations, pharmacy software and technology systems, and insurance.
- Certified Immunizer or willing to become an immunizer within 90 days of hire.
