Clinical Assessment Advisor, Home Health Specialist

As a Clinical Assessment Advisor, you'll play a vital role in shaping the care experience for our patients, working collaboratively with our multidisciplinary teams to ensure seamless transitions and optimal outcomes.
Key Responsibilities:- Conduct comprehensive assessments to identify patient needs and develop personalized care plans
- Collaborate with healthcare professionals to ensure continuity of care and effective communication
- Coordinate care teams to deliver high-quality, patient-centered services
- Monitor patient progress, making adjustments to care plans as needed
- Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
- Bachelor of Science degree in Nursing (BSN) or related field
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role
- Current CPR certification
- Excellent verbal and written communication skills
- Strong interpersonal skills and ability to work effectively in a team environment
- Knowledge of state and federal home health agency regulations and compliance standards
- Ability to read, write, and speak fluent English
- A competitive salary range of $69,800 - $96,200 per year
