About the RoleThis is a unique opportunity to join our team of compassionate professionals who are passionate about making a difference. As a Clinical Assessment Advisor, you'll be responsible for:
- Conducting thorough assessments to identify patient needs and develop personalized care plans
- Collaborating with our interdisciplinary team to ensure seamless care coordination
- Providing education and support to patients and their families to promote health and wellness
- Staying up-to-date with the latest clinical guidelines and best practices to deliver evidence-based care
To succeed in this role, you'll need:
- A Bachelor of Science degree in Nursing (BSN) or equivalent experience
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role
- Current CPR certification and excellent communication skills
- Knowledge of state and federal home health agency regulations and compliance standards
- Ability to work in a fast-paced environment and prioritize multiple tasks
