Clinical Assessment Advisor

We are seeking a skilled Clinical Assessment Advisor to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in ensuring our patients receive the highest quality care in the comfort of their own homes.
Key Responsibilities- Conduct comprehensive assessments of patients' needs and develop personalized care plans to achieve optimal clinical outcomes.
- Collaborate with our interdisciplinary care team to coordinate patient care and ensure seamless transitions between care settings.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care.
- Stay up-to-date with the latest clinical guidelines, regulations, and best practices to ensure compliance and quality care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Bachelor of Science degree in Nursing (BSN) or equivalent experience.
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
- Current CPR certification and valid driver's license.
- Excellent communication, interpersonal, and problem-solving skills.
- Knowledge of state and federal home health agency regulations and compliance standards.
