Key Responsibilities:
- Conduct thorough initial and ongoing assessments of patient needs, identifying areas for improvement and developing targeted interventions.
- Collaborate with physicians and other healthcare professionals to create and update patient care plans, ensuring alignment with individual needs and goals.
- Coordinate care teams to ensure timely and effective delivery of services, promoting a seamless patient experience.
- Monitor patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and seamless care transitions.
Requirements:
- Bachelor of Science degree in Nursing (BSN) or related field.
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
- Current CPR certification.
- Excellent verbal and written communication skills.
- Strong interpersonal skills, with the ability to build effective relationships with patients, families, and healthcare teams.
- Knowledge of state and federal home health agency regulations and compliance standards.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
