We are seeking an experienced MDS Nurse - RN - Registered Nurse or LPN - Licensed Practical Nurse to join our team at D'Youville Life and Wellness Community.
Key Responsibilities:- Interact with healthcare providers, insurance organizations, and families to resolve issues and provide clear communications.
- Ensure safe transitions through care for new admitting, re-admitting, and discharging residents and maximize their Medicare, Medicaid, or other insurance benefits.
- Monitor Point Click Care 'Care Management' areas to ensure accuracy and completeness of resident assessments.
- Monitor ABILITY programs dashboards (InfectionWATCH, RiskWATCH & CareWATCH) and maintain policies for a comprehensive Reimbursement Program.
- Case mix clinical condition tracking.
- Implement auditing mechanisms.
- Oversee and assist with appropriate corrective action of identified deficiencies while reviewing Policies and Procedures.
- Cooperate in surveys and inspections.
- Ensure 672/802 is updated in PCC on a regular basis within required timeframe.
- Review CASPER Reports for accuracy.
- Ensure documentation is retained per home policy.
- Assist in the retrieval of case mix scores.
- Current licensure in the State of Massachusetts as a Registered Nurse.
- Minimum of 1 year of MDS Nurse experience in a long-term care setting.
- AANAC certification (RAC-CT) or the ability to obtain AANAC certification within 6 months of hire.
- Current BLS certification.
